Guidelines for Loss Runs Processing
The following information is required for
ALL requests, for each year requested:

Correct Policy Name (including
DBA's) |
 |
Correct Policy Number |
 |
Correct Policy Effective dates
|
 |
Loss run requests should be sent
to the underwriting or claims department at our office
via fax or by email
|
 |
If you are not the agent for the
account, we will not process a loss run request without
a request on the letterhead of either the Policyholder
(Named Insured) or the Broker of Record (Agent)
|
 |
Loss runs will be processed in
the order received subject to the above
|
 |
Please do not resend requests -
contact our office and we will try to provide a status
|
 |
The loss runs will be either
mailed, faxed (easier), emailed (easiest) to the
Producer requesting. Not the insured (unless otherwise
noted). Make sure to
include all information including phone and fax numbers,
email address, etc. |

will generate a fax request which you can fill out online within
the document, then print and simply fax it back to us!
