REQUESTS FOR COMPANY LOSS RUNS

At H.T. Bailey, we are committed to providing you with exceptional service. To expedite your request for loss runs please review our guidelines noted below. Turn around time can vary depending on the carrier. We will deliver these reports to you as soon as possible. We are usually able to provide loss runs within 2-4 business days of your request, however, be advised that H.T. Bailey is acting as the middleman to the carrier and the time frame could take longer.

Guidelines for Loss Runs Processing

The following information is required for ALL requests, for each year requested:

	Correct Policy Name (including DBA's)
	Correct Policy Number
	Correct Policy Effective dates

	Loss run requests should be sent to the underwriting or claims department at our office via fax or by email
	If you are not the agent for the account, we will not process a loss run request without a request on the letterhead of either the Policyholder (Named Insured) or the Broker of Record (Agent)
	Loss runs will be processed in the order received subject to the above
	Please do not resend requests - contact our office and we will try to provide a status
	The loss runs will be either mailed, faxed (easier), emailed (easiest) to the Producer requesting. Not the insured (unless otherwise noted). Make sure to include all information including phone and fax numbers, email address, etc.
